Registration/ Withdrawal Information

 

REGISTRATION:

In order to register your child at Hopewell, you will need to first fill out a registration form.

Below are the following documents that we need to complete your child's registration.

  • Proof of legal name and birthdate such as a birth certificate or passport
  • Social Security number (requested if available)
  • Tennessee State Immunization Certificate
  • Proof of Residence - (Acceptable proof includes a current utility bill, with the parent's name and address printed on it. If parents have not been living at the residence long enough to have a utility bill, a contract or lease of a house and/or apartment will be accepted.)

 

WITHDRAWAL

In order to withdraw your child from Hopewell, you will need to come by the office to sign a withdrawal form. Please make sure that all library books and textbooks have been turned in. You will also need to make sure that no money is owed in the cafeteria.

REGISTRATION FOR KINDERGARTEN

 

Pre-Registration for Kindergarten usually begins in March.  It is important to pre-register your child before the new school year begins.  This will ensure that our office knows in advance how many kindergarten students to expect. 

 

Below are the following documents that we need at the time of pre-registration:

  • Proof of legal name and birthdate, such as a birth certificate or passport
  • Social Security number (requested if available)
  • Tennessee School Immunization Certificate - (To obtain this certificate, parents should contact their child's doctor or the health department and schedule an appointment for a kindergarten physical.) 
  • Proof of Physical (Children must have either had a physical or have an appointment to get a physical before entering school.)
  • Proof of Residence - (Acceptable proof includes a current utility bill, with the parent's name and address printed on it. If parents have not been living at the residence long enough to have a utility bill, a contract or lease of a house and/or apartment will be accepted.
